Occupational Therapist or Mental Health Nurse
An exciting opportunity has arisen for a registered Mental Health Practitioner to work within an Integrated Community Team as part of the Older Persons Mental Health Service. The team is highly supportive and innovative and aspires to provide a responsive, safe, effective and caring service that is well led.
The post holder will have responsibility for managing, coordinating and delegating work, ensuring referrals are dealt with according to clinical need. Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards. Clinical care at all times will meet acceptable standards of patient safety and quality outcomes. You will manage a varied caseload of service users with an organic or functional mental health diagnosis, providing care in their own homes or in a clinic setting. As an Occupational Therapist the main focus of your work will be to provide therapeutic interventions through individual and group work and to assess for and provide equipment to optimize Activities of Daily Living. You will also manage a small Care Co-coordinator caseload. You will have the opportunity and be supported to develop new skills and interests to enhance patient care and promote professional development.
To be a successful member of our team you will need to be a registered mental health practitioner, have a genuine interest in working with older people and be able to demonstrate a real commitment to providing high quality and compassionate care to patients and their families.
The post holder will be expected to work with a high degree of autonomy and professional accountability and have effective time management skills. You will support the Clinical Team Leads and provide supervision for non-registered members of staff. You will play an important role in the team as we aspire to provide excellent patient care and develop our services in line with national and local drivers.
