Central London Community Healthcare NHS Trust works across nine London boroughs and Hertfordshire. We are rated Good by the Care Quality Commission and are ranked among top NHS employers. Community healthcare is our focus and our passion. We champion the role of community health professionals to make sure our patients get great care closer to home.

Due to an exciting new development we are looking for three new Rehabilitation Assistants to join our renowned Tri-Borough Falls Prevention Service within CLCH. The service sees residents of Westminster, Kensington & Chelsea and Hammersmith & Fulham who are at risk of falls. We see clients in their own homes, and in clinics and deliver evidence based strength and balance classes as well as one to one rehabilitation.

The post holder will work alongside a team of Physiotherapists, Occupational Therapists and other Rehabilitation Assistants and will be supported by their Team Lead and Falls Clinical Lead. The post holder will have a key role in helping to deliver strength and balance classes alongside therapists and will also conduct one to one sessions with clients, working on mobility, balance, improving function and safe use of equipment under specific direction of a therapist. The successful candidates will be given specific training on various outcome measures and screening tools and will conduct these assessments independently in community and clinic settings.

The tri-borough falls prevention service provides an excellent in-service training programme and regular clinical supervision. We pride ourselves on being at the cutting edge of evidenced based falls prevention management and have a national reputation for excellence.

Just as we care about our patients' wellbeing, we care about yours. We offer a range of benefits that include accommodation loans, season travel tickets, childcare assistance, cycle to work schemes and much more.
