Kent Community Health NHS Foundation Trust provides wide-ranging NHS care for people, in their community, in a range of settings including people's own homes; nursing homes; health clinics; community hospitals; minor injury units; a walk-in centre and in mobile units.
We are one of the largest NHS community health providers in England, serving a population of about 1.4 million. We employ 5,000 staff, including community nurses, physiotherapists, dietitians, and many other healthcare professionals.
Our vision is to deliver excellent services which improve the health of our communities. To do this we need to have outstanding people who share a common set of values and aspirations.
Our values (CARE) demonstrate that the Trust acts with integrity and professionalism. CARE stands for:

An opportunity has arisen for a Band 6 Speech and Language Therapist to join this well established and proactive Intermediate Care Team (ICT). You will be part of a multi disciplinary team consisting Occupational Therapists, Physiotherapists, Nurses and Support Workers, providing short term rehabilitation to patients referred into the Team following stroke, and/or exacerbation of long term medical conditions. You will see patients at home, or local care homes, where intervention and advice on communication and swallowing difficulties is needed.

This is a stand alone position within the Thanet team, but with clinical supervision and support as needed from the ICT Clinical Lead Speech and Language Therapist in the Dover/Deal team. You would also be part of a small but supportive SLT team within ICT.

Experience of working closely with individuals requiring dysphagia management and communication impairment intervention is essential. An interest in stroke, experience of working with family/carers, providers, social care and third sector agencies is desirable in order that constructive links can be established and positive health outcomes are achieved, in particular for those patients with complex and enduring health needs. Applicants currently at Band 5 level, looking for a move to a more specialist role, would be considered.
